AUGUST: Drury Lane Convention Center,100 Drury Lane, Oakbrook Terrace, Illinois 60181. Course Instructor: Bruce E. Onofrey, OD, RPh, FAAO Dr. Onofrey, 1982 graduate of the Illinois College of Optometry, is the Chief of Optometry Services and Vice-Chairman of Eye Services at Lovelace Medical Center, Albuquerque, New Mexico. Current faculty appointments include adjunct status at the University of California-Berkeley-College of Optometry, Pacific University, and the University of Houston. Dr. Onofrey is an internationally recognized lecturer on the management of ocular disease and the use of pharmaceutical agents. Accreditation: The course is approved by the Illinois Optometric Licensing and Disciplinary Board as meeting the requirements for prescription and use of oral pharmaceutical agents, and the oral course requirement for license renewal April 1, 2010. CE Credit: Six (6) hours tested and six (6) hours of regular CE credit will be given upon completion of the course. Identification: All participants will be required to present a photo ID at each session to assure proper credit is given. Attendance: Participants must be in attendance for the entirety of each session to receive credit for the course. Test: An on-site open-book examination will be administered immediately following the second day’s lecture. One retake will be permitted in accordance with the law. Parking and Lunch: Parking available free on-site. Lunch provided daily. Deadline: All registrations for either July course must be received no later than July 16, 2008. No on-site registration will be available. Registrations will be accepted on a first come first served basis. Payment: Payment must be received with the registration or the registration will be returned unprocessed. Confirmation: Confirmation and directions will be sent within 10 days of receipt of your registration. If you do not receive confirmation at that time, please call the IOA office. Cancellation: No cancellations will be accepted after July 16, 2008. A cancellation fee of $25 will apply to all cancellations prior to either event.
